AJE wants to remind parents that both HPV opt-out forms as well as religious exemptions for immunizations must be submitted each school year. 

• Religious exemptions and HPV opt-out forms signed from April 1, 2022 through March 31, 2023 expired on July 1, 2023. 
• Religious exemptions and HPV opt-out forms signed from April 1, 2023 through March 31, 2024 will expire on July 1, 2024.

Parents must submit the name of each child, their date of birth, and the school or childcare facility where they are or will be enrolled to: doh.immunization@dc.gov in order to obtain a blank exemption form to complete.  Forms may also be requested in-person Monday Thursday, 9:00am to 3:30pm at: DC Health/Immunization Division Community Health Administration 899 North Capitol Street NE, 3rd Floor Washington, DC 20002.
