Upcoming Events to Highlight New Resource for Health Care Transition Improvements in School-Based Health Centers

Transitioning from pediatric to adult health care is a major step for young people in high school. School-based health centers (SBHCs) are great places to help those students learn to manage their own care, navigate care independently, understand changes in privacy and consent, and find and stay connected to adult care once they leave their SBHC.

 

To help guide SBHC clinicians and other school staff integrate health care transition (HCT) supports into their system, the School-Based Health Alliance and Got Transition have developed the exciting new “School Based Health Center (SBHC) Playbook on Health Care Transition (HCT),” which includes practical strategies that can assist students with their transition from the SBHC to adult health care.

To promote the new SBHC Playbook on HCT, the two organizations are planning two exciting events.

Webinar: Got Transition? Launching Students From SBHCs Into Adult Health Care: A New Playbook for Getting Started

  • The webinar will be held on April 26, 2022 from 3-4:00pm ET and will feature information and guidance for SBHC staff and administrators to help their students transition to adult care. Register today! 

    Twitter Chat: #HCTinSBHCs, hosted by @SBH4ALL and @GotTransition2

  • The Twitter chat will be held on May 11, 2022 from 3-4:00pm ET. It will engage with SBHC clinicians, school administrators, state and local agencies, and high school students and their families, in a conversation on how SBHCs can help students transition to an adult model of care. It will highlight helpful resources and feature lessons learned from SBHCs that implemented HCT resources in their practices.
