Survey to share your family’s experiences aging out of SSI, Medicaid, CHIP, and Title V Programs for Children and Youth with Special Health Care Needs

Got Transition is working on a 2-year project, funded by the Lucile Packard Foundation for Children’s Health, to examine barriers and policy options for youth with special health care needs (YSHCN), particularly Black YSHCN, aging out of four public programs: SSI, Medicaid, CHIP, and Title V Programs for Children and Youth with Special Health Care Needs. They are working with a national advisory group on this effort. More about this project can be found here.

They would like to hear directly from families with young adults ages 19-25 who have or are in the process of aging into adulthood and losing childhood eligibility for the four public programs. It will help them better understand the challenges that Black YSHCN, including those with disabilities, and their families face during this process. Your response will be anonymous. They plan to summarize the anonymous responses to this question in a report, which will include recommendations for how to better support Black young adults and their families as they age out of the four public programs.
