This quote came from a participant in Dc Health Matter’s 2019 Community Health Needs Assessment for the District of Columbia. Thier conversations with over 300+ community stakeholders resulted in a rich collection of 28 top themes in four areas: Mental Health, Care Coordination, Health Literacy and Place-Based Care. The DC Health Matters Collaborative will share more about what DC wants to see the health system tackle related to Health Literacy and Place-Based Care in the last webinar in their four-part series on Thursday, Nov 14 at 2 pm.
A discussion of their assessment findings and examples of our initiatives will be presenters by representatives from Howard University Hospital, Mary’s Center and Sibley Memorial Hospital.
